There are a few different ways to view responses to your form field questions:
Contacts > Client List
- To view answers to form fields for a specific client or participant, click on a client from your Client List
- Make sure you're viewing the correct adult, child or additional adult and scroll down to find Provider Form Fields
- In this section, form fields are displayed in the order they were responded to by the customer
Upcoming > Roster > Download Reports
- To export the answers to form field questions in a spreadsheet for the whole class, click into the roster view for the class from the Upcoming tab
- Click the More Options Menu [...] on the top right of the page to the right of the Message button, click Download Reports, and choose to download the roster for the particular day you've selected or for the week
- You'll receive an email with a link to download a spreadsheet which includes a column for each form field
Upcoming > Roster (Optional)
- From the roster view, click the More Options menu [...] on the top right of the page to the right of the Message button and click Roster Settings
- Scroll to Form Questions, select the form fields to display on the roster page, enter a nickname if you want to display the questions with a shorter label, and click Save
Schedules > Semester or Camps/Events
1. Select a time frame and click the gray Semester Actions or Camp Actions dropdown menu
2. Click Download Reports > Registration Report
Reports > Customers*
1. Scroll down to below the graphs and select the Form Responses Hub Tab
2. On that page you'll see reports of your Form Field questions and the responses. At the top of the page you can use the filter options to filter by the question, response date and more!
It's important to note:
Archived form fields do not appear in the registration report.
Archived form fields should still display in the insights dashboard Form Responses Hub
